8. Expoland, Japan
Fujin Raijin II
The rides' axles hadn't been replaced for 15 years, and in 2007 one of them broke, derailing the coaster and sending cars flying off the tracks. A 19-year-old was killed and 19 others were injured. The park closed that year never to reopen again.
7. Waterworld USA, California
Water Slide
In 1997, 33 high school graduating seniors crowded the water slide, causing the slide to collapse under their weight. One died from a crushed chest, and all 32 others were taken to hospitals. Later, at the graduation ceremony, 17 students accepted their diplomas in wheelchairs.

59-year-old Keith Clarke only cut his toe on a piece of coral. But being a hemophiliac, the wound took longer to heal and became infected. Doctors attempted to save him by amputating his leg below the knee but Clarke died soon after.
5. Six Flags Kentucky Kingdom
The Superman Tower of Power
The ride lifts its passengers about 17 stories and plunges into a free fall. Unfortunately, in 2007, a cable broke, wrapping around 13-year-old Kaitlyn Lasitter’s neck and legs. She managed to free her neck, but as the ride suddenly dropped it snapped taut and severed her feet. Surgeons however, were able to reattach the girl’s right foot.
4. Schlitterbahn Waterpark, Kansas City
Verruckt
Ten-year-old Caleb Schwab tragically fell to his death this year as his raft went airborne and hit the netting over the slide. Other riders had reported earlier that the slide's safety harnesses had not been working properly that day. Caleb was the son of Kansas State Rep. Scott Schwab, and was at the park with his family to celebrate "Elected Official Day."
3. Cedar Point, Ohio
The Raptor
James Young, 45, sneaked past a fenced-off area near the Raptor roller coaster trying to retrieve his fallen cell phone. He was then hit by a speeding car on the ride, dying almost instantly.
2. Berdsk, Russia,
Ferris Wheel
A young lad jumped a six-foot fence to an abandoned ferris wheel, managed to turn it on himself and climbed aboard. He later decided to change seat, scaling the huge structure before falling 45 feet to his death. Police are searching for the onlookers who stood there and filmed it, rather than trying to help.
1. Sealand and SeaWorld
Tilikum
Not surprising that Tilikum the Killer Whale has a murderous streak.
In 1991, Sealand trainer, Keltie Byrne, fell into a pool containing three orcas. Tillikum pulled her to the bottom of the tank and held her there until she drowned.
Tilikum struck again in 1999, when the corpse of a homeless man who was in the park after dark was found the next day, naked, and draped over Tilikum's back. The dead man had torn testicles and various puncture wounds.
And in 2010, Tilikum pulled SeaWorld trainer Dawn Brancheau underwater by her ponytail and she drowned. However, some whale experts have disputed SeaWorld's claim that the whale bit Brancheau's ponytail, noting that the autopsy reported that her left arm had been torn off.
And to close on a lighter note, some accidents can happen no matter how many safety measures there are. (various birds hitting people on rides)
The Italian model Fabio experienced this when visiting Busch Gardens in Williamsburg, Virginia. A goose flew into his face, causing him minor injuries. Unfortunately, the goose didn’t make it.
